Must be at least 16 years of age

Must have a valid Social Security card (Please bring a copy of this document to orientation)
Physical Examination by a physician or nurse practitioner within 6 months prior to clinical rotation (to be paid for by student).
PPD (TB) test within 1 year prior to our clinical rotation (to be paid for by student).
Must pass a Live Scan Fingerprinting background check (will be completed during class and there is no need to complete prior to the course)
