Lost Your Continuing Education Certificate?

If you completed CNA/HHA CEUs with Alpha First Medical Academy, we can email you a replacement PDF. We do not print or mail replacements. A $100 non-refundable/non-transferable administrative fee covers the record search and re-send for an eight-hour class. If you have lost more than one certificate, you will need to add extra tickets at checkout to receive all your lost certificates.

What to include in your request: Full name used at the time (and any prior name), email address used during purchase, email address for delivery, date of class attended. We may request a photo ID to verify identity. Certificates are re-sent exactly as originally issued (no edits to names/dates). AFMA-issued certificates only. If the name or date does not match the sign in sheet, your certificate cannot be re-issued.

Can you mail me a printed copy?

No. Replacement certificates are delivered by email only.

I need multiple certificates. Do I pay multiple fees?
Enter all requested certificates in your submission. The $100 administrative fee applies to one eight-hour course to locate and re-send your AFMA CEU certificate. Therefore, if you have lost more than one day of CEU certificates, you will need to purchase multiple $100 tickets to receive all of your complete CEUs.
I completed CEUs somewhere else. Can you resend those?

No. We only have access to AFMA issued certificates.

My name has changed. Will the certificate show my new name?
We re-send the original certificate as issued. Please include any previous names used so we can find the correct record.
Can you send the certificate directly to my employer or the state?
No. For privacy, we send the certificate to the email you provide.
How far back do your records go?
Records can be retrieved from July 2024 until present.
